Choosing the right doctor for your health needs is important.
You can change your Sharp doctors anytime throughout the year, once per month. Changes must be made by the 15th to be effective the 1st of the following month. Simply, call the SDPEBA Sharp Hotline at 888-840-4747. They can assist with that change.
Once you call in and are assigned to the new doctor, you will receive a new SHARP ID card via US mail within 10 business days. To avoid any delays in receiving your SHARP ID card, make sure that your address on the SAP system is correct.
If your address is correct and you do not receive a SHARP ID card, please contact us and we will re-order one for you.
