No matter which plan you enroll in, you'll receive a Sharp ID card. These cards should be brought to every appointment where you plan to use your insurance.
Receiving your ID Card
Once enrolled your ID card should arrive by mail within in 10 business days.
If you have not received your ID card within ten days of employment, check your address in SAP and make sure it is correct.
- If your address is incorrect, please update it and then email us: firstname.lastname@example.org
- If your address is correct, email us and we will request a duplicate be mailed to you.
In the case that you lose your Sharp ID Card or if it is damaged, email us and request a duplicate.
