How do I get a Dental Plan ID card?

ID Cards for your HMO MetLife Dental plan are sent 10 days after you have selected a provider. This card contains your dentist's information as well as your personal information and should be brought to all dental appointments.

If you have not selected a provider yet, click here. 

If you have selected a provider and it has been 10 days, you can request a new ID card by following the instructions below.

Note: We use whatever address is on file. If you have moved recently or have not updated your address your ID card might be sent to the wrong address. Update your address in SAP before requesting a new one. 

To Request an ID card:

  1. Call 800.880.1800
  2. Enter Primary Enrollee's ID Number (SSN)
  3. Confirm Last Name
  4. Select Option 3 - ID Card
  5. Confirm Mailing Address (If incorrect, hang up. Update with SAP, then call again)
  6. Automated response will confirm request was accepted and card will be sent in 10 business days

PPO Plan

MetLife does not send ID cards for PPO as you do not need one to see a dentist or access services. However, if you would like an ID card you may print one out. To do so you will need to first create an account on the MetLife website. 

  1. Go to https://mybenefits.metlife.com
  2. In the “MetLife Benefits Login” box, enter “SDPEBA” and click Next
  3. If you have already created an account, enter your Username (email) and Password and click LOG IN. Continue to step 5
  4. If you have not created an account, click CREATE A NEW ACCOUNT
    1. Enter your information and click NEXT
    2. Create a password, security questions, and agree to the terms, then click NEXT
    3. Click NEXT
    4. Click GO TO MYACCOUNTS
  5. Under ID cards, Click PRINT

Note: You may come across sections in this process that refer to San Diego Municipal Employee Association. If you are not associated with MEA please ignore and continue with the outlined process.
